Understanding your condition
What Is Upper Back Pain?
Upper back pain refers to discomfort, stiffness, or aching arising from the thoracic spine — the middle section of the spine that runs from the base of the neck to the lower back. This region includes twelve vertebrae, the facet joints between them, the muscles and ligaments of the mid-back, and the points where the ribs attach to the spine.
Unlike the neck and lower back, the thoracic spine has less mobility by design. This means that when stiffness or restriction develops, it can be noticeable fairly quickly and affect how the rest of the spine moves, often contributing to discomfort in the shoulders or surrounding areas. Upper back pain is particularly common in people who spend long hours sitting, working at a computer, or in a forward-flexed position.
At True Chiropractic, we assess the thoracic spine in the context of the whole spine and surrounding structures to understand what is contributing to your symptoms before recommending care.
Common causes
What Can Cause Upper Back Pain?
Thoracic pain often develops as a result of postural loading, repetitive strain, or changes in how the spine moves as a whole. Identifying your specific contributing factors is an important part of the assessment.
🪑
Prolonged Desk Work
Sitting for long periods in a forward-flexed posture places sustained compressive load on the thoracic joints and stretches the muscles of the mid-back, which can lead to pain and stiffness over time.
📱
Screen and Device Use
Looking down at phones or screens for extended periods encourages a rounded upper back posture that can contribute to thoracic joint restriction and muscle overload.
💪
Muscle Tension and Trigger Points
The muscles of the mid-back and between the shoulder blades are prone to developing trigger points and tightness, particularly with repetitive upper limb tasks or sustained postures.
🦴
Thoracic Joint Restriction
The facet joints of the thoracic spine can become restricted or stiff, reducing the normal gliding movement of the vertebrae and contributing to localised or referred pain.
🚗
Previous Injury or Trauma
A history of whiplash, a fall, or impact to the upper back can leave lasting restrictions in thoracic joint mobility and muscle function that contribute to ongoing discomfort.
🏋️
Repetitive Work or Sport
Manual work, swimming, rowing, and other activities that place repetitive load on the thoracic region can contribute to joint and muscle overload over time.
Signs and symptoms
How Upper Back Pain Can Present
Thoracic pain can present in a range of ways depending on which structures are involved. Common presentations we see at True Chiropractic include:
- Aching or stiffness across the mid or upper back
- Sharp pain with certain movements, particularly rotation or extension
- Pain that worsens after sitting for long periods
- A feeling of tightness or restriction between the shoulder blades
- Pain that refers around the ribs or into the chest in some presentations
- Discomfort that eases with movement but returns after rest
- Audible clicking or popping in the thoracic spine with movement
- Associated neck or lower back pain due to compensatory changes
Seek immediate medical attention if: your upper back pain is accompanied by significant shortness of breath, chest pain, fever, unexplained weight loss, or follows significant trauma. These may require urgent medical assessment to rule out non-musculoskeletal causes.
How we approach your care
What Chiropractic Care for Upper Back Pain May Involve
After a thorough assessment of your thoracic spine and surrounding regions, your chiropractor will explain their findings and what they recommend. Care may include:
- Thoracic spinal manipulation or mobilisation to restore movement in restricted joints and reduce pain in the mid-back
- Rib mobilisation where rib restriction is contributing to upper back or chest wall discomfort
- Soft tissue therapy targeting tight muscles in the mid-back, shoulder girdle, and surrounding regions
- Therapeutic exercise to improve thoracic mobility, postural endurance, and the strength of the mid-back and shoulder stabilisers
- Postural and ergonomic advice covering workstation setup, sitting habits, and how to incorporate more thoracic movement into your day
- Education and self-management strategies including targeted stretches and movement breaks to support your recovery between visits
Your first visit
What to Expect at True Chiropractic

- Thorough physical assessment including movement testing, orthopaedic and neurological screening relevant to your presentation
- Clear explanation of findings so you understand what may be contributing to your symptoms and what your options are
- Care plan and first treatment if appropriate and agreed upon at the end of the assessment
Upper back stiffness responds well to the right care. Most patients are seen within the same week.
Common questions
Frequently Asked Questions
Can a chiropractor help with upper back pain?
Chiropractors are trained to assess and manage thoracic spine conditions including joint restriction, muscle tension, and postural-related upper back pain. A thorough assessment will determine whether chiropractic care is appropriate for your presentation.
Why does my upper back click so much?
Clicking or popping in the thoracic spine is often caused by normal joint movement or gas release from the facet joints. In most cases it is not harmful, but if clicking is associated with pain or restriction it is worth having assessed to identify the underlying cause.
Can upper back pain cause pain around the ribs or in the chest?
In some presentations, thoracic joint restriction or muscle tension can refer pain around the ribcage or into the chest wall. However, chest pain should always be assessed medically first to rule out cardiac or other serious causes before attributing it to the thoracic spine.
Do I need a referral to book an appointment?
No referral is required. You can book directly online or by phone. If your GP has referred you under a Chronic Disease Management plan you may be eligible for Medicare rebates on a limited number of visits.
Do you accept private health insurance?
Yes. We have HICAPS on-site so you can claim on the spot. Chiropractic is generally covered under Extras cover. Check with your fund before your visit to confirm your entitlements and any applicable waiting periods.
