Is Chiropractic Care Safe?
One of the most common questions before a first appointment and one that deserves a direct, honest answer rather than marketing language.
Written by Dr Nicholas Lee • AHPRA Registered Chiropractor • True Chiropractic, Heidelberg VIC 3084
The short answer
Is Chiropractic Care Safe? The Evidence-Based Answer
Yes — chiropractic care performed by a registered practitioner following thorough assessment is considered safe for the vast majority of presentations. It is one of the most commonly used allied health services in Australia, used by an estimated 21% of Australians at some point in their lives.
The more useful question is not ‘is chiropractic safe in general’ but ‘is chiropractic appropriate and safe for my specific presentation?’ That determination requires a proper assessment, which is why assessment always comes before treatment at True Chiropractic.
What Are the Actual Risks?
Common mild side effects
The most commonly reported side effects after chiropractic treatment are temporary and mild: localised soreness, stiffness, or fatigue in the treated area for 24–48 hours after a session. These are similar to what you might experience after a massage or a new exercise. They occur in a minority of patients and resolve quickly without intervention.
Serious adverse events
The most discussed serious risk associated with chiropractic is a vertebral artery dissection following cervical (neck) manipulation. This is very rare. A 2010 review in the journal Spine estimated the incidence at approximately 1 in 5.85 million cervical manipulations. Importantly, research suggests that patients who experience vertebral artery dissection are often already in the prodromal (early) stage of the event when they seek care for neck pain, meaning the association with manipulation may be correlational rather than causal in many cases.
Chiropractors screen for contraindications before any cervical technique is applied, including vascular risk factors, neurological signs, and red flag symptoms. If there is any clinical uncertainty, higher-velocity techniques are not used.
Who Should Not Have Chiropractic Manipulation?
• Fracture or bone tumour in the region being treated
• Severe osteoporosis — mobilisation and other non-manipulative techniques can be used instead
• Cauda equina syndrome — a medical emergency requiring urgent surgical referral
• Active inflammatory arthropathy in an acute flare
• Cervical instability (e.g. rheumatoid arthritis affecting C1/C2, Down syndrome)
• Anticoagulant medication — high-velocity techniques are modified or avoided
This is why a thorough intake assessment is not a formality, it is the mechanism through which contraindications are identified and the approach adapted accordingly.
What to tell your chiropractor
What to Disclose Before Your First Appointment
The following information is important for your chiropractor to know before treatment begins. You will be asked about these in your intake history, but be proactive about disclosing them:
• Any history of cancer or unexplained weight loss
• Osteoporosis or bone density concerns
• Blood thinning medications (warfarin, aspirin, newer anticoagulants)
• Previous spinal surgery
• Neurological symptoms — weakness, numbness, bowel or bladder changes
• Pregnancy
• Recent trauma or fracture
• Rheumatoid arthritis or other inflammatory conditions
Disclosing these does not mean you cannot be treated, it means the approach can be appropriately adapted for your situation. A good chiropractor will always work within the parameters of your health history, not around them.
Frequently asked questions
Common Questions About Chiropractic Safety
Is chiropractic safe for older adults?
Yes, with appropriate technique modification. High-velocity manipulation may not be appropriate for patients with significant osteoporosis, but mobilisation, soft tissue therapy, and exercise prescription are all suitable. Many older patients benefit significantly from chiropractic care for chronic back and neck pain, hip pain, and mobility maintenance.
Is chiropractic safe during pregnancy?
Yes, with technique adaptations for each trimester. Prone positioning is avoided in the second and third trimester, and gentler mobilisation techniques are used. Please mention your pregnancy when booking so the room and approach can be prepared appropriately from the start.
Can children see a chiropractor?
Paediatric chiropractic care uses entirely different techniques to adult care — much lighter, gentler contacts rather than the manipulation used for adults. If you are considering chiropractic for a child, discuss the specific presentation and approach with us before proceeding.
What if I am nervous about the adjustment?
This is very common. At True Chiropractic, Dr Lee explains every technique before applying it and will always seek your consent. If you are uncomfortable with a particular technique, alternatives are available. You are in control of your care at every point.
